About the Role:
Join Livingston Memorial Visiting Nurse and Hospice's compassionate team as a Registered Nurse (RN) delivering skilled nursing care in patients’ homes. You’ll care for individuals with a wide range of medical needs — from routine to complex — and make a meaningful impact by helping them reach their highest level of independence.
What You’ll Do:
- Conduct comprehensive nursing assessments (including OASIS) at SOC, ROC, discharge, and other milestones
- Develop and maintain individualized, patient-centered care plans
- Administer medication management and reconciliation
- Educate patients and caregivers to support independence
- Obtain timely physician orders and ensure proper documentation
- Case manage patients, coordinating care across interdisciplinary teams
- Communicate effectively with physicians, supervisors, and care teams
- Supervise LVNs and Home Health Aides (CHHAs)
- Ensure regulatory compliance with Medicare notices (HHABN, NOMNC)
- Participate in quality improvement and training initiatives
- Accurately report hours, mileage, and maintain documentation standards
- Serve as a role model and preceptor as needed
What We’re Looking For:
- Licensed Registered Nurse (RN) in the state of California
- Strong clinical judgment and critical thinking skills
- Current BLS certification.
- Home Health or acute care experience preferred
- OASIS experience highly desirable
- Excellent communication and organizational skills
- Ability to work independently and manage a flexible schedule
- Commitment to delivering high-quality, compassionate care
- One year professional work experience as a Registered Nurse.
